In light of the current rise in COVID cases in New Jersey, we wanted to reach out to each of you to ensure we are all doing our best to keep our players and coaches as safe as possible. Please read through as there are some new rules/regulations for checking in and attending practices.

 

  • All players are to still use the TestAlerts app to check in.

  • Temperatures can now be taken individually with the stand alone thermometer (hands free)

  • Washing of hands, or Hand Sanitizer before entering the gym (new hands free sanitizer in lobby)

  • Keep socially distance while in the gym and bathrooms

  • Wear masks whenever possible

NOTICE:

  • We realize there are many cases popping up in the schools and some schools have closed and are requiring students to self-quarantine.

  • If your daughter/son or yourself has come in contact with anyone that has symptoms or has/had COVID we ask that you;

    • Self-quarantine for 14 days

      •  Individuals should leave the place of self-quarantine only to seek medical care/treatment or to obtain food and other essential items.​

    • Get tested

      • Either way, we ask that you notify us immediately. If tested, please provide the results to us prior to attending practice.

  • Per the State of NJ "Travellers from the states listed on this link are directed to self-quarantine for a 14-day period from the time of last contact within the identified state."  If you or someone close to you has travelled to any of these states, we kindly request you do not attend any of our programs (including private lessons) for a 14-day period and/or get tested. -CLICK HERE-

 

We realize this is a difficult time for everyone and  we are all concerned and hopeful at the same time that things will return to some sort of normal very soon. In the meantime, let's do our best to keep us all safe and healthy.

RETURN TO PLAY GUIDELINES FOR INDIVIDUALS –

Adhere to the most stringent federal, state or local guidelines when participating in any activity.

​

All Players/Participants, MUST read, and agree to the new waiver to be allowed to participant in any event at Xtreme. The new waiver is included in the registration for each program, please be sure to read it  in it's entirety.

Xtreme Volleyball Academy Liability Waiver

​

I acknowledge that volleyball or any sporting event is an extreme test of a person’s physical and mental limits and that my participation in a volleyball event (practices, scrimmages, tournaments, private lessons, etc.) can cause potential death, serious injury, or property damage.   With full understanding of such risks, I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the named persons or entities listed below or others, and assume full responsibility for my participation. I hereby take the following action for executors, my administrators, heirs, next of kin, successors, assigns and myself:

 

A. I waive, release, and discharge from any and all claims/ liabilities for death or personal injury or damages of any kind which arise out of or relate to my traveling to/from or my participation in any volleyball event (defined above),

 

B. I agree not to sue any of the named persons or entities listed below for any of the claims or liabilities that I have waived, released, or discharged herein: and I indemnify and hold harmless the named persons or entities mentioned below from any claims made or liabilities assessed against them as a result of my actions. Named Persons or Entities: XTREME VOLLEYBALL ACADEMY, and their executive director, tournament directors, sponsors, and the owners, officers, directors, employees, sub-contractors, representatives, and agents of any of the above.

 

C. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. It is believed that an individual can be in infected with COVID-19 without their knowledge and be asymptomatic. Xtreme Volleyball Academy has put in place preventative measures to reduce the spread of COVID-19. However, XVA cannot guarantee that I or anyone else will not become infected with COVID-19, including my spouse, guests, unborn child, or relatives. Participation in a XVA's program(s) (beach and/or indoor), related event, or activity, could increase the risk of contracting COVID-19. By signing this agreement, I ACKOWLEDGE the contagious nature of COVID-19 and VOLUNTARILY ASSUME THE RISK that I may be exposed to or infected by COVID-19 by participating in a XVA's program(s), related event, or activity, and that such exposure or infection may result in personal injury, illness, permanent disability, and death to myself, my spouse, guests, unborn child, or relatives. I understand that the risk of becoming exposed to or infected by COVID-19 at an XVA's program(s) (beach and/or indoor), related event, or activity may result from the actions, omissions, or negligence of myself or others, including, but not limited to, XVA's employees, volunteers, and program participants. I UNDERSTAND AND VOLUNTARILY ACCEPT AND ASSUME ALL the foregoing risks related to COVID-19 and accept sole responsibility for any injury or illness that may occur. Further, I UNDERSTAND AND AGREE that this release includes any Claims based on the actions, omissions, or negligence of XVA, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any XVA’s program(s), related event, or activity.

 

D.  Photo Release: By signing this document I also authorize XVA to use my/and or my child’s photo and/or video for promotional purposes in all printed and/or electronic media. I understand and agree that photos/videos may be duplicated, distributed with or without charge, and/or altered in any form or manner without future/further compensation or liability, in perpetuity.
